Ontario Health Teams help patients, families and caregivers connect with local health services that can help.
An Ontario Health Team is a collaboration of health care providers working together as one coordinated team to serve the needs of their local communities and help patients, families and caregivers navigate the health system. It may include health care providers in primary care, home care, hospitals, community agencies, long-term care, mental health and addictions, emergency health services, laboratory and diagnostic services, midwifery services and more.
